Measles Is Surging in the United States — and Health Officials Say It Is Getting Worse

Measles, a disease the United States once declared eliminated, is surging across the country at a pace that has alarmed doctors and public health officials, who warn that falling vaccination rates are allowing the virus to regain a foothold it has not held in decades.

The scale of the resurgence is laid bare in new data reported by the Associated Press, whose graphics show just how bad the measles surge in the US has gotten. Case numbers that once numbered in the dozens each year have climbed sharply, outbreaks have spread across multiple states, and health officials say the trend is moving in the wrong direction, with no clear sign of slowing.

    A Preventable Disease Returns

Measles is one of the most contagious viruses known to medicine. It spreads through the air when an infected person coughs or sneezes, and the virus can linger in a room for up to two hours after that person has left. Around nine out of ten unvaccinated people who are exposed will become infected. Symptoms usually begin with a high fever, cough, runny nose and red, watery eyes, followed by the distinctive red rash that spreads across the body.

For most healthy people, measles is a miserable but survivable illness. For others, it can be far more serious. Complications include pneumonia, severe diarrhoea, ear infections that can cause permanent hearing loss, and encephalitis, a dangerous swelling of the brain. Young children, pregnant women and people with weakened immune systems face the greatest risk, and even in wealthy countries with advanced hospitals, measles still kills a small number of those it infects.

The bitter irony, officials stress, is that all of this is preventable. The MMR vaccine, which protects against measles, mumps and rubella, is among the most effective vaccines in use, with two doses providing around 97 per cent protection. It has been used safely for decades and was the tool that allowed the United States to declare measles eliminated in 2000, meaning the virus no longer circulated continuously within its borders.

Falling Vaccination Rates Are Driving Outbreaks

Health officials point to one central cause of the current surge: vaccination coverage has slipped, in some communities well below the roughly 95 per cent level needed to maintain herd immunity and stop the virus spreading. When coverage falls, gaps open up, often clustered in particular schools, neighbourhoods or communities, and it is in those pockets that outbreaks take hold.

Several forces are behind the decline. The disruption of the Covid-19 pandemic caused many children to miss routine appointments, and not all have caught up. Vaccine hesitancy and misinformation, particularly online, have persuaded some parents to delay or refuse the MMR jab. Exemptions from school vaccination requirements, granted on religious or personal grounds in many states, have also risen, steadily eroding the protection that high coverage once provided.

Imported cases add further fuel. Measles remains common in many parts of the world, and international travel continually reintroduces the virus to the United States. In a highly vaccinated population, those imported cases fizzle out. In a population with growing immunity gaps, each one is a spark that can ignite a local outbreak, and officials say that is precisely what is now happening, repeatedly.

What It Means for Public Health

The consequences extend well beyond the families directly affected. Every outbreak demands an intensive response from already stretched local health departments: tracing contacts, arranging testing, setting up vaccination clinics and communicating with anxious communities. Schools face difficult decisions about excluding unvaccinated pupils during outbreaks, and hospitals must guard against the virus spreading to their most vulnerable patients.

There is also a wider warning in the measles data. Because measles is so contagious, it is usually the first disease to return when vaccination programmes weaken, a canary in the coal mine for the whole immunisation system. Officials fear that the same coverage gaps could allow other preventable diseases, from whooping cough to polio, to stage comebacks of their own if the trend is not reversed.

The response being urged is straightforward: catch-up vaccination campaigns for children who missed doses, clear and trusted public information to counter misinformation, and easier access to clinics for families who face cost, transport or scheduling barriers. Doctors are also asking parents to check their children’s records and to speak to their family doctor if they are unsure whether vaccinations are up to date.

    A Note for Irish Travellers

For readers in Ireland, the surge matters for a practical reason: the United States is one of the most popular long-haul destinations for Irish travellers, for holidays, business and visiting family. Anyone planning a trip should make sure their MMR vaccinations are complete well before travelling, and parents travelling with young children should seek advice from their GP, since infants can be especially vulnerable. Health officials on both sides of the Atlantic are clear about the central lesson. Measles has not changed; what has changed is the level of protection around it. Restore vaccination coverage, they say, and the outbreaks will fade. Allow it to keep falling, and a disease once consigned to the history books will keep making headlines.
